Application Process

  1. Complete EPC Application
  2. Complete EPC Agreement 
  3. Complete Merchant Profile Analysis  
  4. Mail forms (with appropriate signatures) *along with the supporting documents below* to the address at the bottom of the page.

Supporting Documents:

  1. Sole Proprietors:   Copy of Business License or Fictitious Name Filing (DBA)
    Partnerships:         Copy of Business License or Fictitious Name Filing (DBA)
    Corporations:         Copy of Filed, stamped Articles of Incorporation.
  2. A voided check on the business account that is to be used in conjunction with the check processing service (must be imprinted with the business name).
  3. Copies of business bank statements for past two months on the account to be used in conjunction with the check processing service (name and account number must be same as voided check). 
  4. Copies of business financial statements for past two years (tax returns preferred).
  5. Photograph of interior (showing inventory) and exterior (showing signage) of the merchant’s place of business. If business is home based, please provide pictures of interior (showing computer, desk, and inventory, if applicable) and pictures of exterior (showing exterior of home and the address). ****Please do not send in undeveloped film or a disposable camera. ****
